Consent/Waiver Release/ Player Commitment

Below is a consent form giving your child permission to be a part of Real Futbol Club. The consent form allows your child to practice and play in games. It is also a waiver of injury. Parents and guardians understand that Real Futbol Club is not liable for any injuries that may occur while participating in the sport, whether games (home or away) or during practice. It is important to understand that practicing and playing in any sport has a risk of injury, but if done accordingly it will keep injuries to a minimum. It is understood that Real Futbol Club will not be held responsible for any injuries that can occur while safety precautions have been given. Real Futbol Club will strive to provide the safest environment possible.

As part of the Real Futbol Club there is a year long commitment to the club and team. If player or parent decide to leave the club before the season year, there will be a $200 fee and additional fees for the remainder of the months left of their seasonal year. This ensures the teams have a complete roster for games and tournaments.

As the Parent/Guardian of my child, I accept terms & conditions regarding the twelve month commitment.

As the Participating Player, I understand that Real Futbol Club and Affiliates will not be held liable for any injuries as mentioned in the above waiver.

Media Release

In consideration for being allowed to participate in the travel soccer and related activities of Real Futbol Club, I/we give permission to have our child’s photographs/images used by Real Futbol Club for news releases, advertising, brochures, flyers, website, social media and any other media for informational or promotional purposes. I/we understand that the photograph/images will be used to illustrate and promote Real Futbol Club.

Real FC and Covid 19 Waiver

Real Futbol Club encourages players and spectators to follow the guidelines or recommendations established by local and state public health authorities. Members are always expected to gear towards the side of caution in health and safety matters.
